Pledge and Invocation:
 
President Sandy Lane brought the meeting to order at 12:15 PM at Purkey’s Pink Apple with Norm Werkheiser leading the Pledge of Allegiance and Jon Buxton giving the Invocation.

Club News:
 
Ad Buck – Rick Miller - United Methodist Beef Dinner on Oct 26 at Triton, beef, mashed potatoes, corn casserole, pie and ice cream. Nice evening out for $10. Opens at 4:00 PM for take-out , 4:30 PM for sit down dinner.
 
The Club celebrated exchange student Ale’s 18th Birthday, which was Oct 15, with a birthday cake with candle and a round of Happy Birthday.
 
Club Dues
 
Please pay your club dues as soon as possible, if you have not already done so. Thank you, if you have already done so. Cost is $135 for period July 1, 2019 through June 30, 2020. We have already paid the first half year of dues for you to RI and our District, so we would really appreciate it, if those still owing dues pay very soon, so we are not forced to terminate your membership in Dec.  The Club is charged dues from RI and District for anyone on our membership rolls on July 1 and Jan 1.

Committee Reports:
 
Roadside Clean up:  Norm Werkheiser
 
The Fall Roadside Cleanup will take place at 9 AM on Oct 26 at the Fireplace Restaurant Parking lot. Please sign up to help since the more volunteers, the faster the cleanup will go.
 
Nominating Committee:
 
The nominating committee is looking for someone to run for Club President for 2021-2022 to follow Terri Detrick.  This person will be President Elect during Terri’s Presidency.  Please let Sandy, Mike Markovitz or Terri Detrick know if you might be interested in serving our club as President.
 
2019 Harvest and Wine Festival Committee: Ron Furman
 
Paid tickets plus sponsor and Rotary Lounge tickets totaled 2,800, plus vendors and Rotarians means we were close to 3,000.
 
Sponsorships were up from last year, with some first-time sponsors. 
 
8-10 vehicles were still there on Sun morning. Ron Furman and Mike Markovitz finished the clean up on Sun and took down all the fencing.
 
7 people forgot their wine at the hospitality tent that Samantha Maruzzelli has been taking care of.
 
At next club assembly on Nov 7, we will discuss wine festival issues, jobs, changes for next year and clarifications of policies.
 
Membership
 
Robin Rome’s membership application was approved by the Board and she will be our newest member on Oct 23.
 
At our Oct 31 Luncheon, we will have a Rotary Trivial Pursuit Challenge and Halloween hat contest.

Gina Suydam did an outstanding job on handling most of the Harvest & Wine Festival Facebook postings, especially during the 7 days before the Festival when lots of queries were being posted.
 
District News:
 
See the Oct District newsletter for information on the various events and activities happening in District 7410. Please support our fellow Rotarians at other clubs in our District for their fun upcoming activities/events.
 
Steamtown Marathon was Sunday.  Every club has been assessed $50 for the Rotary sponsorship of the marathon.
 
Polio Day is Oct 24.

Our next Meeting will be Thursday, Oct 24 and we will have a Mystery guest speaker who will be talk about Rotary Exchange Student Experiences - Sweden 1965-66.
 
Prior to adjournment, John Proctor led the recitation of the 4-Way Test by the attendees.

New Stroke Program Technology
 
Margy is the Geisinger CMC Stroke Program Coordinator. She has a Bachelor’s degree in Nursing, Certified as Stroke Registered Nurse and 27 years in the nursing field.
 
800,000 people have a stroke each year and the majority are large vessel strokes. It is the 4th leading death cause in NEPA, but can also lead to disability.
 
Signs of stroke:  weakness on one side.  If deeper in the brain stem, it can cause weakness, balance problems, dysarthria (marbles in mouth).  Aphasia (difficulty getting words out) and trouble with peripheral vision can be a symptom of occipital stroke. Bleeding stroke can cause difficulty with swallowing.
 
To help prevent a stroke, you now want to keep your BP below 130/80 and reduce other risk factors.  Risk factors for stroke include: high cholesterol, Afib (30% of Afib does not show on EKG), blood sugar above 100, A1C above 5.7, smoking, obesity, sleep apnea, & alcohol.  Age, sex, race and hereditability are not changeable risk factors.
 
If artery blocked, it can cause brain ischemic stroke. Hemorrhagic stroke due to blood vessel leak or break (can be due to high BP). Basilar artery stroke patients may be unconscious and unable to move below nose.
 
If on the new anticoagulants, you cannot have Tissue Plasminogen Activator (TPA, alteplase), since 6% of such patients can experience bleeding events.  Do CAT scan to rule out bleeding in the brain and then do angiogram and life flight to another facility if cannot give TPA.  There are also other factors that would rule out TPA administration.
 
Large vessel strokes are the most disabling. They may be able to remove the clot from the vessel, especially if they cannot give TPA. Only special neurosurgeons can do these brain procedures to remove clots from the large vessels. Up to 24 hr to remove the clot, but brain cells still dying, so the sooner the better.
 
The doctors have 4.5 hr from symptom onset to give TPA. Studies ongoing to try to get FDA approval to move out to 9 hrs. They may give other meds to thin out blood and try to see what is causing your stroke.
 
EMS is first line of defense to get you the correct help ASAP (1.9 billion neurons destroyed every minute after a stroke, so call EMS immediately) using a stroke assessment scale, then ER (NIH Stroke scale assessment and consult with tele-stroke doctor from Danville), then get you correct treatment and to correct experts. Make sure you are going to a primary stroke center, where they may have 2-8 strokes per day and thus lots of experience.
